Home Health Programs & Therapies
We offer many home healthcare programs to address specific challenges or conditions and improve our patients’ quality of life. Our nurses, therapists, and social workers are all licensed professionals who share the mission of providing compassionate and individualized care to the patients and families who put their trust in us.

Skilled
Nursing
Care is led by our skilled, compassionate nurses who work with each patient, in
partnership with their doctor(s), to develop a custom care plan and provide education.
Patients are also supported by certified nursing assistants that can aid in the patients recovery by assisting with hygiene needs and bathing activities until the patient has resumed ability to perform these tasks safely again.
Skilled nursing care can greatly benefit patients experiencing:
- A chronic health condition that requires skilled care
- Care needs to recover from an illness or hospitalization
- Care needs to support a disability
- Difficulty with daily activities, movement, etc. and/or wound care needs
- And many more care needs to enhance quality of life and recovery

Physical & Occupational Therapy
Patients experiencing mobility challenges due to an illness or injury have access to in-home physical therapy that can improve physical ability, balance and strength, as well as help teach the patient safe ways to use wheelchairs or walkers. Physical therapy outcomes often have the added benefit of the patient experiencing less pain with movement.
Patients can also receive Occupational Therapy Services to help them navigate their home environment safely. Our OTs accomplish this by working with the patient and caregivers to establish safe methods of showering, getting dressed,
toileting, and much more.

Speech
Therapy
Speech therapy is also available to enhance both verbal and non-verbal
communication efforts which can often be compromised following an illness or
major event such as having a stroke.
Our STs are also experts in helping
patients and families with cognitive disorders and can help with techniques to
deal with memory loss and dementia concerns both for the patient as well as
their caregivers.
Another benefit of speech therapy is their expert training in
swallowing challenges, to help prevent choking on foods or liquids which can
lead to pneumonia in some instances.

Social
Support
Our patients' care teams include social workers to provide support in the event of financial or social concerns that interfere with a patient’s health or overall well-being.
Our social workers are well versed in the local community resources
available and can assist with applications and connecting our patients with needed services. They also can be a valuable resource in locating various types
of housing such as Senior Independent Living Apartments or Assisted Living Facilities when patients need additional daily support.
In addition to these
concerns, social workers help patients and their families process the emotions that can accompany health setbacks, new diagnosis or conditions and debilitating injuries.
