Specialized Care at Home
We believe that excellent care should be accessible to everyone in the comfort of wherever they call home. In fact, it’s our specialty. Our team is made up of experts who can provide home-based care for many needs, including*:
- Nursing care after a recent illness, new diagnosis, or new medications
- Nurses who perform wound care, including wound vac management
- Physical therapy to regain strength after a hospital stay, injury, or fall

Paying for Home Healthcare

Medicare covers 100% of home health costs for its beneficiaries. To qualify for home health services, a patient must:
1. Be referred to home health by a physician, a nurse practitioner or a physician’s assistant
2. Have an inability to safely leave home without difficulty due to a medical condition, or cannot leave home without the assistance of another person for safety reasons
3. Require skilled nursing care or therapy
4. Select a Medicare-certified home healthcare agency to provide their care

Other ways to pay for home healthcare services may include the following:
• Medicare Advantage Plans – may have co-pays and out-of-pocket expenses associated; varies by plan
• Veteran benefits – no co-pays, and home health must be ordered by a VA healthcare provider
• Private insurance – may have co-pays and out-of-pocket expenses associated; varies by plan
• Out-of-pocket payment – a patient can pay for home health privately; must have an order from their health care provider sent to home health agency

The Magnolia Approach

Whenever possible, care is designed to improve the patient’s condition, and help him or her regain function and independence. Physical, occupational, and speech therapists are available to provide services to patients as directed by their physicians as part of their individual plans of care.

Skilled Care
Care is led by our skilled, compassionate nurses who work with each patient, in partnership with their doctor(s), to develop a custom care plan and provide education.
Patients are also supported by certified nursing assistants that can aid
in the patients recovery by assisting with hygiene needs and bathing activities until the patient has resumed ability to perform these tasks safely again.
Therapy Programs
Patients experiencing mobility challenges due to an illness or injury have access to in-home therapy that can improve physical ability, balance and strength, or communication, as well as help teach the patient safe ways to navigate their homes and perform daily activities.
Therapy outcomes often have the added benefit of the patient experiencing less pain with movement, and improved life satisfaction.
Medical Social Workers
Our patients' care teams include social workers to provide support in the event of financial or social concerns that interfere with a patient’s health or overall well-being.
In addition to these concerns, social workers help patients and their families ones process the emotions that can accompany health setbacks, new diagnoses or conditions, and
unexpected injuries.
Dealing with a recent hospitalization, illness, or injury doesn’t have to mean suffering with pain, fear, anxiety, frequent ER visits, and other symptoms. We are experts in caring for patients with complex care or rehabilitation needs in a home setting.
